Our team focus
Each Mountain View PACE Interdisciplinary Team (IDT) is comprised of multiple clinical and service professionals, including physicians, nurses, personal care assistants, activities staff, therapists, social workers, dieticians, transportation workers, and spiritual support workers.
Together, the IDT staff ensures every level of care is tailored to each participant’s specific needs and properly provided.

Our integrated approach helps each participant receive daily at-home care and support and, at our Day Center, necessary medical services and much-needed social interaction and engagement.
*Mountain View PACE IDT staff provide and/or authorize services. Participants may be fully liable for the cost of services obtained without the authorization of the PACE Team (except emergency services).
