The Mountain View PACE team of physicians, nurses, personal care assistants, activities staff, therapists, dietitians, social workers, transportation workers, and spiritual support workers provide the basis for how each enrollee’s “Individualized Care Plan” will be administered.

Depending upon your health, well-being, and in-home needs

Your Mountain View PACE Approved Care Plan may include:

There is no charge to you or your family for any drug, service, or care approved by your Mountain View PACE health care team.*

*Costs for Mountain View PACE services may be your financial responsibility, depending upon your Medicare and Medicaid eligibility. If you do not have Medicare or Medicaid, services may be paid privately. Enrollees may always withdraw from the Mountain View PACE program and resume their care in regular Medicare and Medicaid programs.

About PACE, Program of All-inclusive Care for the Elderly

The Program of All-Inclusive Care for the Elderly (PACE) provides seniors with an alternative to nursing facilities and enables them to live in their homes. PACE is also credited with improving the emotional well-being of participants.

With PACE, a team of health care professionals works with each individual to make sure needed services are coordinated. PACE programs get to know their participants and design services to meet each person’s unique needs.

How PACE works

PACE covers Medicare and Medicaid-covered care and services, and other services the PACE team of healthcare professionals decide are necessary to improve and maintain your health.

Note: Individuals may disenroll from a PACE program at any time by notifying the PACE team.

Who can enroll?

Mountain View PACE serves seniors from all walks of life who meet the following eligibility requirements:

  • 55 years and older

  • Live in designated counties served

  • Meet state nursing facility level of care criteria

  • Live safely in the community with PACE services at the time of enrollment

How PACE is paid for?

Mountain View PACE accepts Medicaid, Medicare, and private payments.

  • Medicaid only, no premium

  • Medicare and Medicaid, no premium

  • Medicare but not Medicaid, monthly premium

  • Non-Medicare or Medicaid-qualified, monthly premium

Mountain View PACE participants must receive all needed healthcare, including primary care and special physician services (other than emergency services) from Mountain View PACE contracted providers. *Mountain View PACE participants may be fully and personally liable for costs of any unauthorized out-of-network services.
