Adult Day Health Center

Surrounded by your entire care team, we offer a safe place to share socialization with friends, exercise and wellness, family style meals, and so much more.

24/7 Medial On-call Care

Clinical providers are available 24/7 for any urgent medical issues. They are always just a phone call away.

Transportation

Transportation to all medical appointments & to and from the Day Center is provided. We have a fleet of vans equipped with lifts & trained staff to accommodate special transportation needs.

Collaborating closely with each participant and their family members, our Interdisciplinary Team (IDT) takes a holistic approach to developing the most appropriate and personalized care plan to meet their unique emotional, clinical, social, and spiritual needs. Learn more about our programs.
